A Brief Overview
Accountable for direct patient care and supervision of care
delivery by ancillary staff. Coordinates the Resident Assessment
process to ensure that comprehensive assessments are completed
timely and that each resident has an appropriate individualized
ongoing plan of care. Ensures that the staff complete initial and
ongoing orientation/education according to the policies and
procedures of the organization. Provides leadership utilizing the
organization's mission.
What you will do
- Coordinates, documents, and completes accurate Minimum Data Set
(MDS) assessments. Manages the MDS schedule and assures that the
MDS is completed per state and federal guidelines.
- Completes initial admission care plan on each resident as well
as well as updates care plans and status boards on a daily basis to
accurately reflect the residents' status/care needs. Assures that
care plans meet the requirements of the state and federal
guidelines.
- Works closely with the interdisciplinary team to keep open
communication with families and team members. Advocate for Resident
rights. Leads resident care conferences.
- Provides nursing care in accordance with the facility policies
and procedures within the scope of practice of the Registered
Nurse. Maintains a high level of clinical competency and holds
neighborhood staff to a high standard of performance. Shares
knowledge with staff and serves as a role model.
- Active participant in the QAPI program. Assists with collection
and maintenance of data from OSHA and DOH.
- Assures nursing assistant staff is trained per policy of
facility (CNA certification/medication certification) and that
compliance with the program meets state requirements.
- Daily rounding with staff and residents.
- Completes and monitors scheduling of staff. Provides
supervision of nursing team and gives counseling and positive
reinforcement as needed.
- Responsible for overall compliance and success with the annual
DOH survey process.
- Supports application of the organization's mission, vision,

services to our patients, families, co-workers, physicians,
volunteers, and the communities served.Essential Qualifications
The individual must be able to work the hours specified. To perform
this job successfully, an individual must be able to perform each

Required Education, License/Certification, or Work Experience:
- Registered Nurse (RN) - Board of Nursing An active license in
the state of practice Upon Hire Preferred Education,
License/Certification, or Work Experience:
- 1-3 years in long term care and/or supervisory experience
